Old Pa ent Using SOAP Format
, 1. Introduc on
Nausea and fa gue are among the most common yet nonspecific complaints encountered in
primary care, especially in older adults. In geriatric pa ents, these symptoms o en represent
underlying systemic pathology rather than benign, self-limi ng condi ons. The physiological
changes of aging, presence of mul ple chronic illnesses, and polypharmacy significantly
increase diagnos c complexity. Failure to conduct a comprehensive assessment may lead to
delayed diagnosis, disease progression, and increased morbidity.
This iHuman case analysis focuses on the evalua on of a 65-year-old pa ent presen ng with
nausea and fa gue. The case demonstrates the importance of clinical reasoning, differen al
diagnosis development, diagnos c tes ng, and evidence-based management. Addi onally, it
