History of Present Illness (HPI)
A 22-month-old male brought in by his mother due to concerns about delayed social interac on
and communica on. The mother reports that over the past 6–8 months, J.T. has shown limited
interest in playing with other children at daycare. He tends to play alone, does not ini ate
interac on, and becomes upset when rou nes are disrupted.
The mother notes that J.T. rarely responds to his name, has limited eye contact, and does not
point to objects of interest. He has a vocabulary of approximately 3–4 single words and does
not combine words. He communicates primarily by crying or pulling his mother toward desired
objects. No regression of previously acquired skills has been observed.
There is no history of seizures, head trauma, or chronic illness. Pregnancy and delivery were
uncomplicated. Immuniza ons are up to date.
