Seclusions and restraints = only go in there if they are a danger to themselves or others
Monitoring patients with seclusions and restraints, monitor VS. HR, BP (could the restraints be causing
hypertension), could they harm themselves in seclusion with an object, could the seclusion or restraints
be causing a worse behavior? Check blood sugars because blood sugars could be exacerbating the
behavior!! THEY NEED TO BE PHYSICALLY OK!
Pt documentation needs to be really good! Explain the reasoning really well of why they are in seclusion
or restraints. Make sure to be very detailed and show that you tried everything possible first before
putting them in seclusion. Can you read your notes and see that it paints a picture of that being the only
thing you could do for them.
Document the psyche of a patient who is in seclusion because they could be regressing or have flash
backs of past trauma or PTSD.
Document why you let the patient out and how you let them out.. was their behavior better and were
they not a danger anymore to anyone etc..

You want to know the predictors of violence so you can try to de escalate the situation before it gets any
worse.
