homes and over the phone.
DMG picks up where the PCP's capabilities leave off. Our team works along with your PCP to
provide excellent care right in the member's home.
For in-home visits, we can identify any living conditions concerns that may impact that
member's health.
This service is FREE of charge to Devoted Health Plan members.*
In-home services help keep patients healthier, happier, out of the hospital and in their homes.
