What is the most common complaintwith the menstrual cycle, and complications; Painful menses (dysmenorrhea) Know STI’s and risks. Manage dysmenorrhea non-pharmacologically with heat, diet – eating properly, exercise, getting rid of excess fats, alcohol, carbs. Pharmacological interventions include NSAIDS, Motrin, pamprin, Midol – include diuretic that deals with edema.2. If a patient finds a breast lump how will you advise her? Report lumps and follow up with a doctor, ultrasound or mammogram. Early recognition is the key for less invasive treatment.3. What is your role in the GYN exam – review STIs. There are bacterial, viral, protozoan, bacterial treated with antibiotics. Viral – HIV, HPV, Herpes – treat symptoms and address lifestyle – diet and stress – maintain immune system – keep viral load down.4. Contraception - Forms of birth control – condoms, spermicides, birth control pills, Nexplanon, IUD, emergency contraception, vaginal rings, Mirena, Depo Provera, diaphragm, patches, abstinence. good subject for your postpartum patients as well – know them, know the risks and some of the educational pointsto share with them regarding each type. Many items do not protect against STIs, IUD is long term – meant fora monogamous relationship – can come out, Depo requires injections every 12 weeks & suggests calcium supplements – birth control should be taken same time every day. Smoking is contraindicated with Birth control. Choices should be based on someone’s lifestyle, breast feeding is contraindicated with estrogen-based birth control – impedes prolactin in breast milk. Remember birth control choice should be based on a patient’s lifestyle – if she cannot swallow pills do not offer oral contraception – right? Etc…5. Emergency contraception – know education. Within 72 hours, not 100% effective, not for birth control, does not cause an abortion, does not protect from STIs, encourage follow up and testing for STIs afterwards. 6. Preconceptual care is provided for a means to identify risks and provide nutrition – not to establish who should become pregnant. Access is important, counseling identifies risks- lifestyle, nutrition, immunizations, genetic issues, prenatal vitamins, smoking, drugs, alcohol, provides opportunities for community referrals.7. Pregnancy – understand the structure and function of the external and internal genitalia. Uterus – house growing fetus, ideal musculature for delivering baby, menstrual cycling. Ovaries – house eggs and produce estrogen (growth hormone) and progesterone (most important - quieting hormone) pro gestation, pro-life hormone. Fallopian tubes – carry the eggs from the ovaries to the uterus. Egg is fertilized by the sperm in theampulla. Bladder is involved in pregnancy – can cause complications during pregnancy and delivery. Know the purpose of each in the process of pregnancy. You must understand the changes that occur to each system - 8. Please identify pregnancy history – G, F, P, A, L – this mnemonic is only for delivery - know how to complete this given each women’s obstetrical history. G=Gravida, number of pregnancies, F=full term pregnancies, P=preterm pregnancies before 37 wks, A=abortions, L=Living children. Twins are only one pregnancy(gravida) and one delivery(parity). 9. EDD – Nagles rule – 1st day of LMP - 3months, + 7days. Based on a 28-day cycle, does not work for everyone, can be a few days off.
