Service Plan Brief for Broward Health Out Patient Wound Care Clinic
Proposed Nursing Service Idea
To create a wound care clinic at Broward Health outpatient facility for patients that need
help managing and healing chronic wounds. The center's purpose will be to meet the needs of
community members affected by chronic wounds. Services provided include wound assessment
by licensed providers, dressing changes, medications, and dietary modification education.
Clinicians consist of physicians, registered nurses, medical assistants, and dieticians. Non-
clinical staff includes office manager and receptionist. With this diverse staff, practical
leadership skills using active listening, decision-making, effective communication, and
flexability are essential. The Complexity Leadership Model (CLM) can be utilized as the
leadership approach to implementing the wound care center. This model is not authoritarian but
focuses on collaboration, expanding knowledge, and leading to action (WGU, n.d.). The model
allows leaders to collaborate with other leaders and their staff when sharing ideas and
implementing changes. By working together as a team, leadership may successfully implement
the new wound care center for patients at Broward Health outpatient clinic.
Importance
Many community members have diabetic foot ulcers or chronic venous stasis ulcers that are non-
healing. The center will assist members of the community that are in need in managing these
chronic wounds. It is essential since these wounds cause significant distress in patients' quality of
life by interfering with their activities of daily living. People living with chronic wounds describe

the experience as isolating, debilitating, depressing, and worrisome, all of which contribute to
high levels of stress (Van Den Kerkhof, RN, 2016). It is imperative to create more options for
patient's with chronic non-healing wounds requiring wound care to improve healthcare in our
community. There is a growing incidence of chronic wounds and recognition of the complexity
involved in caring for these patients. The prevalence of chronic wounds is estimated at 2% of the
United States population (Kim et al., 2013). Wound care has become an essential prominent field
in healthcare.
