A. The healthcare industry faces a persistent challenge in providing preventative care access
to underserved communities. Many individuals within these communities experience
significant barriers, including but not limited to a lack of healthcare understanding, financial
constraints, and little to no transportation availability. Addressing these barriers can also
address the need to reduce preventable hospital admissions, lower healthcare costs, and
improve population health outcomes.
This project proposes a hospital-driven community partnership model to bridge the gap in
preventative care access. Hospitals in the underserved community can extend their reach
beyond physical locations by collaborating with local organizations, colleges, healthcare
providers, and policymakers. In doing so, we can provide mobile health clinics, telehealth
services, and community-based education programs. This initiative ensures a proactive
approach to healthcare delivery, emphasizing early intervention and disease prevention.
The feasibility of this solution is supported by evidence from existing community health
initiatives that demonstrate cost-effectiveness, improved patient engagement, and
enhanced health outcomes. By leveraging hospital resources, integrating community
partnerships, and utilizing digital health solutions, this model presents a sustainable and
scalable solution to an urgent healthcare administration challenge.
B1. Due to the lack of preventative care access among underserved communities, there are
often increased emergency department visits, incidences of chronic diseases that are left
untreated or even undiscovered, and higher healthcare spending. The project proposal
ensures that hospitals would be able to extend their services beyond just acute care and
foster a more community-centered approach to healthcare delivery. The project proposal
is not only focused on providing the proper healthcare needs for an underserved
community but will also focus on providing opportunities for those within the community
through sponsorships, clinical hours for medical students at the local colleges, and
volunteer hours as needed.
