II. Reason for Referral
Timothy is being referred to due to having learning difficulties in school. Timothy is

noted as having average grades. However, Timothy's grades fluctuate tremendously

throughout the school year. Timothy is being referred due to having difficulties with

organizing school tasks and his schoolwork area at school and home. Also, additional
concerns are he is currently struggling with grammar and vocabulary.
Timothy is being evaluated for a possible learning disability and/or potential (ADHD)
Attention-Deficit/Hyperactivity Disorder. A Neuropsychological evaluation will be

administered to make a proper diagnosis. Most neuropsychological evaluations are

given to infer a conclusion about the structural and functional characteristics of a
humans brain by evaluating a person's behavior in a specific stimulus-response
situation (Wu, Anderson & Castiello,2002).

III. Current Symptoms/Presenting Concerns
Th
Timothy has difficulty organizing his work for school, which includes homework and
his workspace. Timothy seems to have trouble with tasks with multiple steps and
tends to skip or leave out necessary steps. Timothy's grades are average, but they tend
to vary a lot. Timothy has always had problems with grammar and vocabulary.
Currently, Timothy is experiencing conflict with his math teacher. Timothy has
always loved mathematics and solves problems without showing his work; this causes
friction with the teacher as the teacher wants Timothy to show his work. The teacher
has given implications that Timothy may be cheating, which causes combative
behavior between Timothy and his teacher.
