Male Genitourinary SOAP Note Form
SUBJECTIVE
Chief Complaint: The patient complains of urinary hesitancy, with post-urination
dribbling gradually worsening over the last two months. The patient states he has
nocturnal voiding at least three or four times a night despite reducing fluid and
caffeine intake before bed. The patient denies urine color or odor changes,
dysuria, or discharge.
History of Present Illness: The patient states he was previously told his prostate was
enlarged.
Review of Systems:
General: No Acute distress.
HEENT: Patient states no vision or hearing changes, nasal discharge or congestion,
sore throat, or difficulty swallowing.
Cardiac: Patient denies chest pain or palpitations.
Pulmonary: Patient denies shortness of breath, cough, or recent respiratory
infection.
GI: Patient denies abdominal pain, nausea, vomiting, diarrhea, constipation, or
changes in stool.
GU: Patient states urinary hesitancy with post-urination dribbling, which has
gradually increased for two months. The patient denies pain, burning, or
discharge.
Musculoskeletal: Patient denies muscle weakness or joint pain.
