Evaluation
A 67-year-old female was readmitted to Nightingale Community
Hospital due to a possible postoperative wound infection. In initial
admission, the patient had received a laparoscopic hysterectomy that
had to be converted to an open procedure due to complications. Due
to fever and drainage, the patient was readmitted seven days ago. She
received surgery five days ago to treat an abscess that developed after
her previous surgery and a central line was placed for administering
long-term antibiotics.
After reviewing the Nightingale Community Hospital Surgical
Patient Tracer Worksheet for this patient, a deficiency can be identified
in the admission assessment. The worksheet identifies that a history
and physical was not done within 24 hours of admission, it also notes
that the history and physical was completed after 72 hours. History and
physicals should be completed by the end of shift when admitted and
within 24 hours of admission.
