Affidavit - Answer-An affidavit is a sworn statement of facts made under oath. It should always identify
the state and county where it is signed and have the signature of a notary public or other person having
authority to administer such oaths.
Alternative - Answer-An affidavit which provides the facts relating to the service of the process identified
in it (or inability to serve it). Should identify the documents served and state the date, time, place and
manner of service. If service is made by a person other than the sheriff or a deputy sheriff, the return
must be by affidavit.
Certificate of Service - Answer-A statement made under penalty of perjury, but not notarized, which
provides the facts relating to the service of process related to a particular case.
Conformed Copy - Answer-An exact copy of a document on which has been written explanations of
things that could not be or were not copied. A written signature might be replaced on conformed copy
with a rubber stamp or notation (/s/)indicating that it was signed by the person whose signature
appears on the original.
