The healthcare system care system attends to patients with a wide range of health complications,
from different racial and ethnic backgrounds and age groups. As such, there has been a growing
focus on patient care over the years calling for the application of appropriate care plans that are
aimed at achieving the treatment outcomes. It has become necessary to adopt an interdisciplinary
approach while offering patient care that entails aspects such as effective communication,
patient-centered care, team work and effective leadership. Nurses have been charged with the
duty to come up with plans to ascertain that the patients’ desires and needs are met accordingly
(Yakaryılmaz & Öztürk, 2017). Teams bring together their resources and knowledge towards
ensuring patient’s recovery. As such efficient treatment outcomes can be achieved through the
application of a collaborative treatment plan (Qiu et al., 2018). This avoids the application of
similar treatment approaches and minimizes the risk of readmission particularly for the elderly
patients. The focus of a care plan that focuses on the geriatric population is to control the effects
of the disease and prolong their life.

The patient is a seventy-nine-year-old White woman named, Mary Jenkins. The patient is
forgetful and suffers from Diabetes. She lives in an elderly’s home and is in need of specialized
care to manage her condition. The patient was diagnosed with Diabetes II in 2017 but the
medical symptoms had manifested one year prior to the diagnosis. The symptoms included
extreme hunger, blurred vision, increase thirst and weight loss (Yakaryılmaz & Öztürk, 2017).
