How does the CRMA transcribe new orders? - ANSWER-Once the order
is received, the CRMA will read the order and assure the order is
complete with all the required components. The CRMA will write the
medication, dose, time of administration, route, and indications on the
MAR in black or blue ink only. The CRMA will assure that the date and
time to begin to administer the medication is clearly indicated, times
administered should coincide with facility times as much as possible. A
stop date should be indicated if appropriate. The CRMA will fax the new
order to the pharmacy and note the order. Another CRMA will review
the transcription and second note the order
Can a highlighter be used on the MAR? - ANSWER-Yes - only for specific
reasons: blocking out days prior to start date of new meds, for
discontinued meds "noting medication discontinued, CRMA initials and
date" - this may be written in red ink.
Do not use the highlighter to highlight empty boxes, it should only be
used to indicate discontinued meds and for dates medication not
ordered for administration.
Can discontinued medications remain in the medication cart or
medication bin? - ANSWER-NO - all discontinued or expired medications
