Chief Complaint: "I have this sharp pain in my lower right side, and I
feel like I'm going to throw up."
SUBJECTIVE (S)
History of Present Illness (HPI):
Tristan Taylor is a 28-year-old male who presents to the Emergency
Department with a chief complaint of acute onset right lower quadrant
(RLQ) abdominal pain that began approximately 12 hours ago. He
reports that the pain initially started around his belly button
(periumbilical region) and was dull and crampy in nature. Over the
past 6 hours, the pain has migrated to the right lower quadrant and
, has become sharp, constant, and more severe, rated 7/10 on the pain
scale.
He reports associated nausea with two episodes of non-bloody,
non-bilious vomiting over the past 4 hours. He also endorses a
subjective low-grade fever and chills. He denies any diarrhea,
constipation, blood in stool, or urinary symptoms (dysuria, frequency,
hematuria). He reports a decreased appetite since the pain started. He
denies any recent travel, sick contacts, or similar symptoms in the
past.
He has tried taking Tylenol at home with minimal relief. He reports the
pain worsens with movement and coughing and is somewhat relieved
by lying still with his knees bent.
Review of Systems (ROS):
● General: Reports subjective fever, chills, fatigue.
● Gastrointestinal: RLQ abdominal pain (sharp, constant), nausea,
vomiting (x2). Denies diarrhea, constipation, hematochezia,
melena.
● Genitourinary: Denies dysuria, urinary frequency, urgency, or
hematuria. Denies testicular pain or swelling (if applicable).
● Musculoskeletal: Denies back pain or joint pain.
● Neurological: Denies headache, dizziness, confusion.
● Skin: Denies rash or jaundice.
Past Medical History (PMH):
