Preeclampsia: HTN that starts after 20w gestation
-Mild: BP: 140/90, protein in urine, headache, swelling in hands and feet, sudden weight gain
-Severe: BP 160/110; greater # protein in urine, severe headache, blurred vision, EPIGASTRIC
pain (right upper quadrant is liver); effects liver-HELLP; LIVER is priority, intrauterine growth
restriction of fetus
HELLP: Hemolysis, elevated liver enzymes, low platelets
Severe Preeclampsia management: Magnesium Sulfate (antidote-
Calcium gluconate) Mag Sul prevents seizures from occurring, IF this is givenseizure precautions
needed; normal S/E: flush face and feeling of hot, S/E: decreased respirations, decreased urine
output, increased sedation; burning at IV site, sweating A/R: decreased deep tendon reflexes,
blurred vision, slurred speech, severe muscle weakness, Cardiac arrest BP and HR decrease.
Therapeutic level Mag Sulfate: 4-7meq/L
Preeclampsia normal BP: 130/90 is ok
Eclampsia: Seizure has occurred-lay pt on side and nothing goes into mouth, suction, oxygen
Protein in urine: +2 or +3 needs to be reported to MD immediately
Station/location: Accommodations to growing fetus and allows passage for the infant through
the birth canal; Stations -3,-2,-1,0,+1,+2,+3; Negatives are above the ischial spine; Zero is at the
ischial spine Pregnancy Terms:
Gestations: age in weeks
Trimester: 13 1/3 weeks long
Gravida: number of pregnancies (regardless of gestation time) including present one
Pre-term: 20-36w6d
Term: 37 -42w
Abortion: any lost pregnancy prior to 20w(regardless of how or why)
Naegele’s Rule: first day of last period, +7 days, -3 months= due date Rubella: If mom not
immune, can only be given after childbirth, right before discharge
Leakage of fluids: Notify MD-not normal (membrane could have ruptured)
Preterm labor: common causes-advanced maternal age, obesity, smoking, gestational diabetes
Shortness of breath: Normal is active, but not normal when at rest-Notify
