History of Present Illness (HPI)
25-year-old female presents with a 4-day history of a pruritic rash. Rash began on
the trunk and has spread to bilateral arms and neck. Patient describes lesions as
red, slightly raised, and itchy. Denies pain, burning, or drainage. No blistering. No
prior similar episode.
Denies fever, chills, sore throat, joint pain, or recent illness. No new medications.
Reports using a new scented body wash 1 week ago. No recent travel. No known
sick contacts. No known insect bites.
Pruritus worsens at night. No relief with over-the-counter lotion.
