HISTORY OF PRESENT ILLNESS (HPI)
A 25-year-old female presents with the onset of a new, pruritic rash on both
forearms and dorsal hands that began ≈ 24 hours ago.
• Onset: Sudden onset yesterday morning.
• Location: Bilateral forearms, dorsal hands, and lateral elbows. Sparing
palms, soles, face, and trunk.
• Quality: Multiple erythematous, raised, well-demarcated plaques (1–3
cm), some with tiny vesicles at the edges. The rash is warm to
touch, non-purulent, and intensely itchy (rating = 8/10).
• Duration: Persistent; no improvement.
