Chief Complaint: “I have this weird rash on my chest and back.”
SUBJECTIVE
Chief Complaint (CC)
“My skin broke out in a rash about a week ago.”
History of Present Illness (HPI)
A.B. is a 25-year-old woman who presents with a new rash on her trunk for 7
days.
• Onset:
– Noticed a single large patch on her left upper chest about 7 days ago.
– 3–4 days later, multiple smaller spots appeared on her chest and back.
• Location:
– Starts on left upper chest.
– Now involves chest and upper back.
– No involvement of face, scalp, palms, soles, or mucous membranes.
• Character/Quality:
– Describes rash as slightly raised, pink/salmon-colored, itchy but not
painful.
– Itch is mild to moderate, worse with heat and sweating.
• Severity:
– Itch severity 4–5/10.
– No disruption of sleep.
