2. Chief Complaint
Sudden weakness of the right side of the body and difficulty speaking
for 2 hours.
3. History of Present Illness
Mr. John was well until 2 hours before admission when he suddenly
developed:
• Weakness of the right arm and leg
• Slurred speech
• Drooping of the right side of the face
• Difficulty holding objects
• Mild headache

His wife noticed he was unable to speak clearly and brought him to the
hospital immediately.
4. Past Medical History
• Hypertension for 12 years (poorly controlled)
• Type 2 Diabetes Mellitus
• Hyperlipidemia
• Smokes 1 pack/day for 30 years
No previous stroke reported.

• Amlodipine
• Metformin
• Atorvastatin
