SUBJECTIVE
History of Present Illness:
Camilla Franklin is a 48-year-old female who presents to the clinic with
complaints of persistent fatigue and increasing irritability that have been
ongoing for approximately two to three months. She describes the fatigue
as constant, overwhelming, and generalized, significantly impacting her
ability to perform daily activities at work and at home. She notes that the
fatigue is unrelieved by rest or sleep. Her irritability is characterized by
increased sensitivity and frustration, particularly with family and coworkers,
which has caused some interpersonal strain.
The patient reports that symptoms began gradually and have progressively
worsened. She endorses associated symptoms including difficulty
concentrating which she describes as mental fog, difficulty staying asleep
through the night, mild headaches, and unintentional weight gain of
approximately five to six pounds over the past few months despite no
significant changes in diet or physical activity. She also reports dry skin,
cold intolerance, occasional hair thinning, and decreased libido. Her
menstrual cycles have become irregular over the past six months,
occurring every six to eight weeks.
She denies any fever, chest pain, palpitations, shortness of breath,
dizziness, or recent illness. She denies depression, anxiety, or suicidal
ideation.
