Week 4 & 5 Lecture Notes
• Processes of Birth
• 800 mL (vaginal) to over 1000 mL (c-section) of blood loss is considered a
hemorrhage, 500 mL is considered a normal amount of blood loss
• 5 P’s of Labor and Birth —> Passenger, Passageway, Position, Powers, and
Psyche
- Powers: help to move the baby down, contractions and mom pushing
- Passageway: the pelvis
- Passenger: the fetus, the placenta, the membranes, passenger has to
be in the correct position (you want baby in cephalic position, curled
up, and facing moms back), fetal lie is important in determining if it
can be a vaginal delivery or a c-section
- Position: the position of the mother during birth (this was on ATI and
Edapt, but may not be on our exam)
- Psyche: anxiety, culture and expectations, birthing experience,
support, the emotional response
- All of these are interrelated and have to work together, as nurses we
want a good outcome, sometimes we are the mom’s only support
person
- Keep an eye on dad if you can, they might pass out
• `Dilation, Effacement, and Station
- Descent: movement of the fetus through the birth canal
- Engagement: fetal presenting part reaches 0 station, when the baby
reaches the ischial spines of the pelvis
- -1, -2, and -3 (anything above the ischial spines) the baby is ballottable
- Flexion: flexes head inward
