Client Scenario
0900 Nurses' Notes:
An 82-year-old female is admitted from a long-term care facility with a 3-day history of
severe diarrhea, poor oral intake, and generalized weakness. Her medical history includes
heart failure and osteoarthritis (treated with daily NSAIDs). The nursing home staff reports
she has not voided in the last 12 hours.
Physical Assessment:
General: Lethargic but rousable to voice. Dry mucous membranes and poor skin
turgor.
Vital Signs: T 99.1°F, HR 108, BP 92/58, RR 20, SpO2 95% on room air.
Cardiovascular: Weak, thready peripheral pulses.
