STUDY CLASS
CASE
---
**Course:** NR576 (Advanced Physical Assessment)
**Patient:** Victoria Lewis, 25-year-old Female
### 1. History of Present Illness (HPI)
, - **Chief Complaint (CC):** "I have an itchy rash on my inner thighs and
left forearm."
- **Onset:** The rash began roughly 24-48 hours after returning from a
weekend trip to Napa Valley.
- **Context:** During her trip, Victoria went hiking off-trail in wooded
areas ("bushwhacking") wearing shorts and a tank top. She later used a
hot tub with her boyfriend.
- **Quality/Location:**
- **Inner thighs (bilateral):** Itchy red bumps.
- **Left inner forearm:** Linearly distributed blisters.
- **Associated Symptoms:** The affected areas feel tender and
swollen. The itch is intense, described as disrupting her sleep.
- **Self-treatment:** She has not taken any medications or applied any
creams prior to presentation.
- **Possible Triggers:** She suspects insect bites, contact with poison
ivy/oak during the hike, or a reaction to a new body lotion or scallops
(which she ate for the first time).
- **Timing:** The development of lesions occurred about 1 day after
the hike/hot tub exposure, suggesting a delayed reaction.
### 2. Review of Systems (ROS)
- **Constitutional:** Denies fever, chills, night sweats.
- **HEENT:** Denies eye redness, swelling, or oral lesions.
- **Respiratory:** Denies difficulty breathing, cough, or wheezing.
