1. Immediately upon discovery, all liquified petroleum gas licensees shall notify the department
of any liquefied petroleum gas or related accident involving a liquified petroleum gas company
or customer account which:
a. Caused a death or personal injury requiring professional medical treatment;
b. resulted in the uncontrolled ignition of liquified petroleum gas; or
c. caused estimated damage to property exceeding $3,000
