how do you pay for the exam? How much does it cost? - ANSWER-$1,025.00
You can pay at the time of application with VISA or mastercard or by cashiers check
or money order with the 10 digit CRDTS number written in the lower left hand corner.
Personal checks can not be accepted
Who provides liability insurance for CRDTS and for how much? - ANSWER-$1-3
million is required
The medical protective company of Fort Wayne, IN in cooperation with CRDTS
no action or payment is necessary for the student
When is the application deadline for CRDTS? - ANSWER-approx 40 days before the
date of the examination
When can an applicant be disqualified from the exam? - ANSWER-The program
director can be disqualified for any reason without discretion it just has to be sent in
writing via email before the start date of the candidates scheduled examination.
If you cancel your exam do you get a refund? - ANSWER-If you cancel at least 30
days prior to exam you get a full refund and if it is 6 days prior you get a 50% refund
If you fail you can re test at a future site and you must complete a new application for
how much money? - ANSWER-$1,025.00 and all portions of the failed exam must be
re taken not just the procedures with deficient performance.
, All candidates who are unsuccessful with their first exam attempt will be eligible for
the onsite retake option, expect for candidates who are unsuccessful for the
following reasons: - ANSWER-1.) unprofessional conduct
2.) Critical tissue trauma
If the student fails, when does the student have to notify the CRDTS coordinator of
their intent to retake? - ANSWER-by 12:00pm for the morning group
by 5:00pm for the afternoon group
there is NO fee for on site retakes and no additional documentation is needed
NO remediation is needed after 1 failed attempt but some states require remediation
after the 2nd failed attempt
