A: FIELD EXPERIENCE AGENCY
I was permitted to conduct my experience in the field at Huntsville Family Medicine.
Huntsville Family Medicine is a team of physicians and employees who are committed to
providing high-quality medical care while preserving personal, hometown atmosphere. HFM is a
medical specialty which provides continuous and comprehensive medical care for people of all
age groups. Furthermore, the healthcare providers of HFM and their patients are being
encouraged so that they can be collaborators in their healthcare decisions (HFM, n.d.).
B: CLIENTS, INTERACTIONS, AND CARE COORDINATION EXPERIENCES
, Bottomfield is a 39-year-old white female with a history of anxiety and depressive
disorder. In addition, the patient is married with dependents of various ages. Bottomfield is a
current some days smoker and currently drinks alcohol. The patient arrived at the clinic for an
annual wellness and a complaint of increased anxiety disorder. I the writer was able to assist the
patients’ medical provider with during the patient annual examination and conduct the patient’s
general health assessments. Additionally, I the writer was permitted to assist with escribing the
patients’ medications to the patient’s local pharmacy. Furthermore, Bottomfield was advised to
resume medications and return to clinic if not having good control of symptoms or go to the ER
with any SI/HI. Otherwise, patient counseled about importance of maintaining health blood
pressure and healthy cholesterol level. In addition, the patient advised per provider to have a
routine screening for colorectal cancer, depression screening, and mammogram with a pap smear
at next annual physical examination.
Karen, Gale is a 73-year-old white female with a personal medical history of
hypertension, hyperlipidemia, hypothyroidism, and Type 2 diabetes mellitus with hyperglycemia.
During this encounter was able to assist the provider during this patient annual wellness
examination with general wellness assessment. The patient was counseled via the provider
regarding the importance of maintaining health blood pressure and healthy cholesterol level, and
diabetes screening and continued surveillance. Furthermore, the patient was referred to
Huntsville Memorial Women's Center for annual mammogram screening advised to follow-up
with her specialty provider for endoscopy and colonoscopy within the next 2-3 years. Advised to
return the clinic in 6-12 months for follow-up examination.
C: CLIENT HEALTH HISTORY

hypertension, hyperlipidemia, hypothyroidism, and Type 2 diabetes mellitus with hyperglycemia.
