Reason for Visit: High blood pressure (HTN)
Past Medical History:
o Known history of hypertension (on/off medication?)
o Possible cardiovascular risk factors (hyperlipidemia,
diabetes)
Medications: May include antihypertensives (ACE inhibitors,
diuretics, beta-blockers)
Allergies: NKDA (unless specified)
Family History: Cardiovascular disease, hypertension
Social History: Smoking, alcohol, exercise, diet habits
Symptoms:
o Headache, dizziness, visual changes (blurred vision)
o Chest discomfort or palpitations
o Fatigue, shortness of breath
o Possible edema in legs
