Chief Complaint
"My blood pressure was high at a health fair last week"
History of Present Illness (HPI)
The patient is a 57-year-old male with no significant past medical history who presents
for evaluation of elevated blood pressure discovered incidentally at a community health
fair approximately one week ago, where his BP was 160/100 mmHg. He reports
occasional mild occipital headaches, typically in the morning, which he has experienced
intermittently for several months. He denies any associated chest pain, shortness of
breath, palpitations, vision changes, dizziness, or leg swelling.
He was told years ago that his BP was "borderline," but no medication was prescribed,
and he did not follow up. He takes no regular medications and has no known drug
allergies. He reports no recent illnesses, stress changes, or significant lifestyle changes.
