Social Security Act- Section 1861(u) specifies the following - Hospitals
are PROVIDERS:
- Critical access hospitals (CAHs)
- Skilled nursing facilities (SNFs)
- Home health agencies (HHAs)
- Hospices
- Comprehensive outpatient rehabilitation facilities (CORFs)

the following are included as providers if they meet the
requirements of Section 1861(p)(4)(A): - Rehabilitation agencies
- Public health agencies
Community mental health centers (CMHC) are providers of services for partial
hospitalization services only.
Suppliers that must meet conditions for coverage (CFCs) to Supplier conditions for coverage (CfC's) include the following:
participate in Medicare include:
- Portable x-ray services
- Rural health clinics (RHCs)
- End stage renal disease (ESRD) facilities
- Physical therapists in independent practice (PTIPs)
- Occupational therapists in independent practice (OTIPs)
- Ambulatory surgical centers (ASCs)
- Organ procurement organizations (OPOs)
, There are no conditions for coverage or surveys - Durable medical equipment (DME) suppliers
performed for:
- Pharmacies
- Prosthesis suppliers
- Laboratories that must meet CLIA requirements & that participate in Medicare are
considered suppliers.
Federally Qualified Health Centers (FQHCs) Recognized as suppliers of services and provide Medicare Part B coverage for
outpatient services similar to those provided by Rural health clinics (RHCs)

and enforcement regulations identify FOUR types of
surveys: - Abbreviated standard survey
- Extended survey
- Partial extended survey
