1. Based on the review of the history, physical and lab findings what is the most likely

diabetes diagnosis for this patient?


Based on the patient's history, physical, and lab findings, it can be concluded that the patient

has Type 2 Diabetes. The patient has been experiencing fatigue, weight loss, extreme thirst,

and increased appetite, which are consistent with Type 2 Diabetes (Mayo Clinic, 2023). He also

has a history of obesity, hypertension, and hyperlipidemia. Since Type 2 has a stronger genetic

association compared to Type 1, it is evident that this disease runs in his family. The patient's

lab findings are also consistent with Type 2. His A1C is 7.2%, and his glucose (fasting) is

132mg/dL, which is higher than the normal range of 60-120 mg/dL. Type 2 Diabetes is also

common in 40 or older adults with sedentary lifestyles, which the patient is.


2. Explain the pathophysiology associated with the chosen diabetes diagnosis.


The pathophysiology of Type 2 Diabetes is a common metabolic disorder affecting older

adults globally. Type 2 Diabetes is primarily caused by two main factors: defective insulin

secretion by pancreatic β-cells and the inability of insulin-sensitive tissues to respond to insulin

(Galicia-Garcia et al., 2020). Once the body is resistant to insulin, the pancreas will no longer

produce insulin, which limits the body's capacity to maintain glucose levels. Defects in the

feedback loops between insulin action and insulin secretion lead to high glucose levels in the

blood or hyperglycemia, resulting in the progression of Type 2 Diabetes (Galicia-Garcia et al.,

2020).


3. Identify at least three subjective findings from the case which support the chosen

diagnosis.

Subject findings from the case study that are seen in Type 2 Diabetes are fatigue, weight

loss, extreme thirst, increased appetite, gaining more weight, and insomnia due to having to get

up and urinate more than four times at night. Mayo Clinic (2023) states that these symptoms are

consistent with Type 2 Diabetes.


4. Identify at least three objective findings from the case which support the chosen

diagnosis.


Objective findings that are consistent with Type diabetes based on the case study are the

individual's lab results, which show that his A1C is 7.2 and glucose (fasting) is 132. The normal

A1C is below 5.7%, and less than 100mg/dL glucose (fasting) is considered healthy (Mayo

Clinic, 2023). Other objective findings are the patient's weight of 240 lbs., BMI of 36.5, and age

of 48. Type 2 Diabetes is common in adults 40 and older, overweight, and obese.
