Week 2 Study Guide Questions

1. On the drug concentration curve, what is the first sign of a therapeutic effect?

On the drug concentration curve, the first sign of therapeutic effect is the point at
which the drug has started to produce the intended effect. “The onset of action”

2. Describe the purpose of the blood-brain barrier and the fetal placental barrier.

The blood brain barrier is a barrier made-up of semi permeable endothelial cells located
in the brain. The barrier acts as a shield for the brain, protecting the brain from many
drugs. This person it protects the brain from drugs entering the CNS. Only drugs that are
non ionized and lipid soluble can pass through the barrier.

The fetal placental barrier is a shield that protects the fetus from the mother's blood, it
is easier to pass than the blood brain barrier. For drugs that passed through the barrier,
they only must use simple diffusion. All drugs that are lipid soluble can pass through this
barrier.

3. What is the difference in distribution for each medication route?

The two different routes of administration include enteral (oral) and parental
administration(IV, IM). Non ionized oral medications are diffused passively from lower to
higher concentration-GI tract to the blood. When oral medications are absorbed, they
go to the liver where they can be metabolized and distributed into the bloodstream.
Routes such as IV, IM and sublingual bypass the GI tract and liver making distribution
quicker and therefore making the onset of action occur quicker. PH of a drug can affect
the distribution. “The state in which a drug exists half in its charged state and half in its
uncharged state is called the acid dissociation constant or pKa.”

Volume of distribution amount of drug in the body divided by the plasma drug
concentration. Explains volume of drug in a particular body compartment.

If drugs are bound by protein they are not going to be able to exert their efforts.

4. How will renal insufficiency affect drug elimination?
