- **Sex:** Female (could be adapted to male; this case uses female)
- **Chief Complaint:** “I can’t stop scratching my skin, especially at
night. It’s been going on for months.”
## 2. History of Present Illness (HPI)
**Onset & Duration:**
Gradual onset over 3 months; worsening in the last 2 weeks.

Initially only on flexural areas (elbow creases, back of knees). Now also
involves neck, wrists, and ankles.
**Quality:**
Described as intense, intermittent “crawling or burning” itch. No pain.
**Aggravating Factors:**
- Heat, sweating, wool clothing, stress, night time.
- Dry winter air.
- After showers (especially hot water).
**Relieving Factors:**
- Cool compresses, moisturizers (brief relief).
- Antihistamines (loratadine) slightly help but not completely.
**Timing:**
Worse at night; interferes with sleep. No clear seasonal variation but
worse in winter.
