SBAR Situation • Patient is currently breathing at 30 breaths per minute with audible wheezing in the chest. Patient has received 500 mcg dose of ipratropium in a nebulizer. Patient states he is feeling better however there is still audible wheezes. Background • Ms. Hoffman is a 33-year-old brought into the ED by her neighbor. She has a history of asthma and has had multiple ED visits within the last year. She is in respiratory distress and struggling to breathe. She can only answer with one work statements and she has an infusion of normal saline. Assessment • Based on the patient’s vitals I believe she needs steroids to assist her breathing. Currently the vital signs are HR:125, BP: 135/79, RR: 32 and SpO2: 83%. Recommendation • I recommend the patient be prescribed 100 mg of methylprednisolone which is a steroid to correct treatment of severe asthma.
