a) The first is a subpoena to testify, which is an order to a
witness to appear and testify at a court proceeding or
deposition. The subpoena tells the witness the name of
the court, the title of the case, the number of the civil
action, the party at whose instance the witness is
summoned, and the date, time, and location where the
witness is to appear.
b) The second is a subpoena to produce evidence
(formally called a subpoena duces tecum), which is an
order to bring specified evidence (which may include
designated documents, records, books, electronically
stored information, or other tangible things) to a court
proceeding or designated person. This subpoena tells the
witness the name of the court, the title of the case, the
number of the civil action, the party at whose instance the
evidence is to be produced, specific items to be produced,
and the date, time, and location of the court proceeding
where the items are to be brought.

uses one form (AOC-G-100) that can serve as a subpoena
