I took the patients vitals and asked the appropriate questions regarding his issue. I made the patient feel at ease with a toy and offered him a drink. Scenario Analysis Questions1 EBP What priority problem did you identify for Charlie Snow? I identified that the patient has contact dermatitis most likely due to the detergent that his aunt used to wash his clothes. EBP What complications might Charlie Snow face if his allergy is not recognized and actions are not initiated in a timely manner? If left untreated, the patient can start having difficulty breathing, he could start wheezing, his throat would be itchy and it would feel like its closing up. Further delay can lead to death. He should also make an adult aware if his face or throat starts to feel funny, puffy or tingly, if he has trouble breathing or anything that feels weird and not normal for him. For his Aunt she should always keep Benadryl on hand for an allergic reaction. She should give him a dose of Benadryl and wash him immediately with soap and cool water to remove any possible allergens from his skin. She should make sure she stays with Charlie to monitor his symptoms. If he starts to swell or state that he has trouble breathing she should call 911 right away. I would then ask the aunt to give me feedback about what I just told her so that I know she understands the steps to take. S/I Because Charlie Snow is accompanied by his aunt and not by a parent, what legal matter must be considered before performing treatment? The aunt must provide a document of consent signed by his parents that she has the right to make medical decisions on Charlie’s behalf. T&C Why is it important that Charlie Snow’s aunt communicate with Charlie’s teacher, school nurse, and principal about his allergies? What should she communicate? The school needs to be aware of Charlie’s allergies to ensure that he is not exposed to them. The nurse needs to be prepared to handle the situation should he come in contact with or ingest an allergen. Charlie should carry an epipen if his allergy is life threatening. QI Describe some actions that should take place to improve Charlie Snow’s quality of care. Care should be taken to have a child friendly atmosphere so that Charlie feels safe and at ease. He should be spoken to in terms that he is able to understand using pictures or dolls to explain procedures or symptoms. His aunt should be taught how to handle a child with allergies and what to look out for and when to call 911. Concluding Questions Reflecting on Charlie Snow’s case, were there any actions you would do differently? If so, what were these actions, and why would you do them differently? I think I did a pretty good job, however I forgot to show Charlie the faces chart and ask him how he was feeling. I also forgot to do teaching to him and his aunt. Describe how you would apply the knowledge and skills that you obtained in Charlie Snow’s case to an actual patient care situation. For an actual patient I know what exactly to look for now and say to the patient to get an understanding of how they are feeling. I am aware of the proper teaching techniques for a child and for the parent or guardian. Children need to be spoken to in a certain way so that they can understand what’s asked of them and how to handle situations. The parent or guardians also need to be taught how to handle emergency situations and the steps to take should things get worse.
