Chief Complaint
“Shortness of breath that has been getting worse over the past 3 days.”
History of Present Illness (HPI)
Robert Coleman is a 72-year-old male with a history of hypertension and ischemic heart
disease who presents with progressively worsening shortness of breath for 3 days. He
reports difficulty breathing when lying flat and now sleeps using three pillows. He wakes
up at night gasping for air.
He also reports:
• Swelling in both legs for 1 week
• Fatigue and reduced exercise tolerance
• Mild dry cough
• Weight gain of about 2–3 kg over a week
He admits he may have missed some of his “water pills” recently due to feeling well.
No chest pain currently, but mild pressure sensation earlier in the week.
