board the following information relating to a disciplinary action taken during the
preceding three years regarding a person regulated by the board:
(1) the identity of the person;
(2) the nature of the complaint that was the basis of the disciplinary action
taken against the person; and
(3) the disciplinary action taken by the board.

with a reasonable opportunity to appear before the board and to
speak on any issue under the board's jurisdiction.
