Guided Reflection Questions
Opening Questions
How did the simulated experience of Red Yoder’s case make you feel?
This simulation made me feel more confident. I was already familiar with Mr. Yoder and his condition, so
I knew what the priority assessments were.
What nursing actions did you feel were appropriate within this scenario?
I believe assessing his orientation and mental status were important, completing a head-to-toe
assessment, taking his vitals, and assessing and caring for his wound
Scenario Analysis Questions*
PCC What is the correlation between Red Yoder’s open foot wound, the diagnostic findings, and
treatment modalities including fluid bolus, insulin, and antibiotic therapy?
Mr. Yoder had an open foot wound that had become infected and was evidenced by an elevated
WBC count. Mr.Yoder was also dehydrated and given a fluid bolus to rehydrate him and also
received antibiotics to treat the infection from wound. He received insulin and given to control
his diabetes and correct his glucose levels. Mr. Yoder had not been in proper control of his
diabetes and makes him more likely to acquire peripheral neuropathy, and this is what makes
wound healing difficult because he can’t feel his wound. All are correlated because if his
diabetes is not in control he is at risk for infection and ulcers and will need medication for the
diabetes and for the infection. The diagnostic findings help confirm presumptions of
uncontrolled diabetes and infection.
PCC What potential problems can you identify for Red Yoder if his symptoms are not managed in a
timely manner?
Mr. Yoder is at risk of sepsis if condition exacerbates, amputation if necrosis or spread of
infection takes place. These risks can be irreversible and really put him at risk if not controlled
immediately.
S/QI For the following Safety/Quality Improvement measures, what nursing actions should be
taken during Red Yoder’s case?
Safety/Quality Improvement Measures Nursing Actions
Medication administration Assessing knowledge of medication use, ability to
take his medication, allergies, take vitals prior to
medication administration. Have patient repeat
