,Patient Information
• Name: Emma Rodriguez
• Age: 13 years old
• Gender: Female
• Chief Complaint: “My skin has been very itchy.”
This pediatric dermatologic case focuses on evaluating a teenager with persistent itching and
skin irritation. The purpose of the examination is to determine the cause of the itching, assess
severity, identify possible triggers, and rule out serious skin or systemic conditions.
1. History of Present Illness (HPI)
Emma Rodriguez is a 13-year-old female who presents with complaints of itchy skin that started
approximately 2–3 weeks ago. The itching initially began on the flexural surfaces of the arms
and behind the knees but has gradually spread to portions of the neck and wrists. The patient
describes the itching as persistent and worse at night, causing difficulty sleeping and frequent
scratching.
She reports that the rash appears dry, red, and occasionally flaky. Scratching temporarily
relieves symptoms but often causes worsening irritation afterward. The patient denies pain,
burning sensation, bleeding, or purulent drainage from the affected areas.
Emma states that symptoms worsen after hot showers, sweating, and wearing certain fabrics
such as wool. She reports partial improvement when applying unscented moisturizer but no
complete resolution. She denies recent use of new soaps, lotions, detergents, perfumes, or
medications.
The patient denies fever, chills, weight loss, joint pain, oral ulcers, or recent illness. No recent
travel, insect bites, or sick contacts are reported. She denies exposure to known allergens or
chemicals.
Her mother reports a history of “sensitive skin” since childhood and occasional seasonal
allergies. There is also a family history of eczema and asthma in close relatives.
The itching has started interfering with school concentration and sleep quality due to nighttime
scratching.
