One rule is to always include “the worst-case scenario” in your differential diagnosis and
make sure you have ruled out this possibility based on your findings and patient assessment.
Your goal is to minimize the risk of missing unusual or infrequent conditions such as
meningococcal meningitis, bacterial endocarditis, pulmonary embolus, or subdural
hematoma that are particularly ominous.
What are the "red flags" in this case?
Migraines are very common and mainly in younger women. Another red flag would
be recent head trauma, blurry vision, and/or dizziness. Cluster headaches are extremely rare
and occur more in middle-aged men. Headaches occurring due to an intracranial mass would
be higher on the differential list than cerebral infection or brain-bleed in this patient,
especially since no recent trauma occurred. Symptoms preceding her headaches align with
characteristics of migraine with aura.
Some general red flags that should raise concern in any differential diagnosis include:
Sudden onset of severe symptoms (e.g., "worst headache of my life" → subarachnoid
hemorrhage)
Altered mental status or loss of consciousness
