Instructions:
This worksheet will be used to assess the current PHMS EHRS system to support transition to
a value-based model and assure appropriate data security and integrity. The worksheet
includes the following components:
* assess the purpose and goals of updating the EHRS and HIT requirements to align
with the transition to a value-based model
* address usability and interoperability concerns
* address data security and integrity concerns
* draft a phased implementation plan
This worksheet will form part of the HIT strategy for Paducah Health Medical System that you
will create in the assessment.
Purpose and Goals
1. Analyze the transition from a volume-based to value-based model. What EHRS-
specific goals should be implemented to support a value-based model? Consider the
domains of safe, effective, patient-centered, timely, efficient, and equitable.

Usability and Interoperability Concerns
2. Identify strategies to streamline the interface. Consider the end-user needs you
assessed in the HIT recommendation.
a. What architecture strategies should be discussed to support integration of
information among the various departments at PHMS?
b. What architecture strategies should be discussed to support integration of
information between PHMS and Lourdes Hospital?
Emphasis on effectiveness and quality of healthcare would be the driver in
evaluating HIT requirements both internally and between businesses. This will take
the lead to meet the important use of requirements. (Miller, 2020).
. Determine strategies for mobile monitoring. For those same end-user needs, what
strategies will support integration of patient information with mobile monitoring devices
and telemedicine requirements?
Due to value-based healthcare is data-driven, a combination of all data coming into
the system will be vital. The collection of the data will lead to reimbursement
agreements and population management. It will be essential to make sure the
mobile applications are created to integrate directly into the IT structure. (O’'Dowd,
2020)
Evaluate strategies for registries between departments. What strategies will
support patient registries that are interoperable between the various departments at
PHMS? Between PHMS and Lourdes Hospital?
The data retrieved from patient registries will be utilized to support evidence-based
healthcare. The patient registry would obtain data from all clinics and sources
(imaging, lab, mobile medical devices, claims, etc.) and be distributed between
Lourdes Hospital and PHMS. (Leavy, et al., 2019)
. Determine strategies for quality improvement. What strategies will support that
data used for quality improvement and risk stratification can be shared between the
various departments at PHMS? Between PHMS and Lourdes Hospital?
Interoperability utilizing a shared patients’ identifier will safeguard that data be
able to be shared between clinics within Lourdes Hospital and PHMS.
Consider complaints and underlying issues. Consider the stakeholder complaints
identified in the PHMS profile.
a. In what way has the current EHRS system at PHMS led to these complaints?
b. How will a new EHRS system address the underlying issues for these complaints?
The unavailability of continuous integration among department systems is a
concern. The PHMS EHRS system has been mended together little by little causing
Healthcare providers to gather data from several areas to put together a full picture
for patient care. The new EHRS system will guarantee that all clinics can identically
access patient data.
