S – SUBJECTIVE (History & Presentation)
Chief Complaint (CC):
“Extreme irritability, frequent tantrums, and difficulty calming child.”
History of Present Illness (HPI):
A.M. is a 22-month-old female toddler brought by her mother due to concerns of persistent
irritability, frequent crying spells, and difficulty with emotional regulation over the past
several months.
The caregiver reports:
• Frequent tantrums occurring multiple times daily
• Episodes triggered by minor frustration (not getting objects, transitions, hunger, or
fatigue)
• Difficulty soothing the child once upset
• Sleep disturbances (difficulty settling, frequent night waking)
• Hyperactivity-like behavior (constant movement, inability to stay still)
• Limited eye contact at times
• Delayed speech development compared to peers
There is concern for:
• Emotional dysregulation beyond developmental expectations
• Possible neurodevelopmental disorder (e.g., early autism spectrum traits or regulatory
disorder)
No history of:
• Seizures
