Renewal Requirements
Pay fee $60-180, including $120 currently. Complete 48 hours of continuing education, with 6
hours in ethics. Renew within 12 months of expiration by paying an additional $100 late fee.
Requirements for Request Military Inactive Status
Include name, license number or social, date active service begins, and request for inactive
status.
Surrender of License
Submit a written request. Not allowed to reapply for at least 1 year.
Assumed Business Name
Notify director in writing at least 30 days before using the assumed name.
Report of Actions
A producer must report any administrative action or criminal prosecution within 30 days,
regardless of jurisdiction.
Change of Business/Contact Info
Notify director within 30 days of any changes.
Suspend, Denial, Revoke, Refuse Renewal of License
Director may take action for up to 12 months. Applicant has the opportunity for a hearing.
Director must provide 10 days notice of the hearing.
