Blumenthal, D., Abrams, M. & Nuzum, R. (2015). The affordable care act at 5 years. The
New England Journal of Medicine, 372. 2451-2458. doi.10.1056/NEJMhpr1503614.
Blumenthal, Abrams, and Nuzum are three authors of this article. This article brings up
more information about the Affordable Care Act (ACA) which was enacted in 2010 by the
president Obama. It is probably the most important accomplishment of the Obama presidency,
and it could affect the future of the healthcare in the United States. The ACA changed access to
healthcare, the cost of healthcare and the quality of care (Blumenthal et al, 2015). With the
enactment of the ACA, the healthcare systems in the country saw an influx of patient volume that
made daily operations challenging and forced healthcare systems to readjust their staffing,
facilities, and resources. According to the author, ACA overlooked the need to reform the health
care system to compel the cost but still provide quality care. The problem that many are facing
with the Affordable Care Act is individuals gain health care coverage along with patient financial
responsibility. Per the authors’ research, since 2010, the numbers of uninsured people who
gained coverage under the ACA ranged from 7.0 million to 16.4 million; the newly insured were
very pleased that they have health insurance coverage. This article explores more about the
effect on availability of affordable health insurance and access to care, the ACA, and the health
care delivery system. I have found this article very educational and informative as I learned a lot
about the ACA via this article.
, Britton, J. R. (2015). Healthcare reimbursement and quality improvement: Integration
using the electronic medical record: Comment on “fee-for-service payment - an evil
practice that must be stamped out?” International Journal of Health Policy and
Management, 4(8), 549–551. doi.org/10.15171/ijhpm.2015.93
This article is very informative, and it enlightens more about the different avenues and
reimbursement perspective in healthcare. I am very much interested in learning more about the
quality, quantity, and complexity of care nurses provide. I believe the main goal of the hospital is
to provide quality care as it effects the reimbursement as well as image of the hospital. According
to the author, there are different reimbursement mechanisms such as fee for service capitation,
pay for performance, and diagnosis-based payment. Current reimbursement mechanisms have
both strengths and weaknesses. It is also very important to not falsify the data to gain
reimbursement, but data accuracy is very important. “Obtaining and processing information
required to ensure the proper functioning of any reimbursement mechanism requires the
investment of substantial time and effort, and limited human resources have likely contributed to
the inefficiency and even failure of mechanism implementation in various settings. However,
more optimal use of emerging HIT, especially the electronic medical record (EMR), could
enhance the accuracy, efficiency, and ultimate feasibility of any reimbursement system” (Britton,
2015).
