3. HISTORY OF PRESENT ILLNESS (HPI)
Grady Turner is an 18-month-old previously healthy male presenting with acute
onset cough and noisy breathing.
Onset: Sudden (nighttime, ~1–3 AM)
Duration: 1–4 days, worsening
Character:
“Barking,” “seal-like” cough
Inspiratory noise (stridor)
Course:
Began with mild URI symptoms (runny nose, low fever)
, Progressed to nocturnal cough + respiratory distress
Severity:
Worse at night and when agitated
Improves with upright position or humidified air
Associated symptoms:
Low-grade fever (~38°C)
Rhinorrhea
Irritability
Decreased appetite
Negative symptoms:
No choking episode
No cyanosis
