Objectives I have been identified with case management systems - ANSWER continuity of

Case manager, Corp. agency - ANSWER structurally, case management system includes
two basic components.
case manager - ANSWER A person who acts to coordinate services for the individual cli-
ent by serving as a human link between the person being helped and the service system.
Core Agency Functions - ANSWER Special coordinating power and authority. The court
agency for a specific geographic area is responsible for developing contracts with providers
for the delivery of specified services for case manage clients. Examples are negotiating
agreements among other agencies, controlling phones that allow needed services to be pur-
chased for case manage clients.
Functions that comprise case management - ANSWER Assessment, planning, linkage,
monitoring, and advocacy
Case management functions(cycle of steps to identify and responsibilities of vulnerable indi-
viduals) - ANSWER 1. Screening and assessment (initial and comprehensive levels of as-
sessment).
