The nurse is performing a head-to-toe physical assessment on a 9-year-
old patient.
Which approach is the most appropriate when assessing this patient?
-Answering as many of the child's questions as possible
-Telling the child's mother to wait in the lobby during the assessment
-Allowing the child to interact with the equipment during the assessment
,-Making sure that the child complies with all instructions given -
ANSWER ✔✔Allowing the child to interact with the equipment during
the assessment
When assessing pediatric patients, the nurse should use caring,
supportive, yet firm approaches. Children may be more cooperative if
they are allowed to interact with the equipment used during the
examination process as shown in the image below. Pediatric patients are
often more cooperative when a parent or caregiver remains in the room
during the assessment process. While it is important to answer the
child's questions, it is appropriate to follow up if an answer is not
possible during the examination. Compliance to instructions is important,
but it is not a priority when approaching a pediatric patient during a
physical examination.
An older adult states, "I'm afraid I will no longer be able to live
independently. I am so upset with God for all of the things that have
recently happened."
Which response by the nurse should help to address these concerns?
, -"Would you like me to contact our religious or spiritual department for
you?"
-"You know that God really isn't angry with you; sometimes things just
happen together."
-"I'm sorry that all of these things have happened to you, but you may
still be able to live independently."
-"Growing old can be difficult due to all of the changes that occur, and
I'm willing to listen." - ANSWER ✔✔"Would you like me to contact our
religious or spiritual department for you?"
Older adults who feel like they are being punished by God may benefit
from a referral to spiritual or religious counseling. Telling the patient that
they know God is not really punishing them minimizes their concerns.
Telling them that they may still be able to live independently may not be
an accurate statement and may provide false reassurance. While
offering to listen is often helpful, the response that addresses the
patient's statement would be to offer a spiritual or religious referral.
During a patient interview, the nurse asks the patient why they came to
the triage center at the mental health facility.
This question should be considered as which type of interview question?
