Family Health Assessment Part II Komalpreet Kaur Grand Canyon University: Family-Centered Health Promotion NRS-429VN September 20, 2019 Health starts in at home by eating well, staying active, and getting recommended health screening by a health care provider. Health is also determined by access to the social and economic conditions of individuals. Social determinants of health (SDOH) are conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks (WHO, n.d.b)."The model considers individual factors and how these interplay with factors within the environment and community" (Falkner, 2018, par.23). Each factor can improve or worsen the overall health. These factors are not the situation or influenced by induvial, but by the conditions or circumstances, they live in. SDOH can be used to determine family health assessment, age- appropriate screenings, a health model to assist in creating a care plan and outlining family- centered health promotions. The Iran family interviewed economically stable, head of the house-father and all three kids have a job, they are financially stable. Education vise father is a high school graduate, the mother has a language barrier, a twenty-nine-year-old daughter is a teacher, a twenty-six-year-old daughter is register nurse and twenty-one-year-old son is in university studying machinal engineering. Social and Community context, family live together as one unit even children are over eighteen. This family has a good reputation except the mother is always worried about a son and deal with stress and the son deals with anger management. The healthwise mother is diabetic, and the father is obese. They all have health insurance; daughters have insurance through work, parents and son are covered by covered California. The neighborhood is safe with a very minimal crime or violence and has access to food and housing. This family has the opportunity to make healthy choices to live a long, healthy life regardless of income, education .........................................continued..............................................
