How to reinstate an inactive license? - ANS May not return to active status for 2 years. Must
complete the following to return:
- File with board
- pass OH laws examination for each license
- $140 reactivation fee
What are the qualifications to get a CCO? - ANS - 18yrs of age
- No bad crimes
- Completed crematory operation certification program
- CCOs who are actually in charge of the facility do not have to be licensed embalmers/FDs
Where are funeral home/embalming facility/crematory license and embalmer/director licenses
and CCOs displayed? - ANS In a conspicuous place within the facility. Additionally, the name
of the FD/embalmer/CCO to whom the funeral home license has been issued shall be displayed
immediately on the outside or the inside of the primary entrance used by the public.
Funeral homes should have one of the following: - ANS Embalming room equipped,
maintained, clean, used only for embalming, preparation, and holding of the dead and
containing articles, facilities, and instruments necessary for those purposes. (Embalming
facilities also need this).
