Introduction
This examination is designed to assess the comprehensive clinical competency of graduate nursing
students in advanced health assessment. The NU 650 Final Comprehensive Exam evaluates proficiency
across 16 interconnected domains that reflect the full scope of advanced practice nursing assessment skills
required for safe, evidence-based patient care.
The examination consists of 100 multiple-choice questions, each with four answer options (A through D)
and one clearly identified correct answer. Each question is accompanied by a detailed clinical rationale
that references current evidence-based guidelines, validated screening tools, and authoritative clinical
references including Bates' Guide to Physical Examination and History Taking, the American Association
of Colleges of Nursing (AACN) Essentials (2021), United States Preventive Services Task Force
(USPSTF) recommendations, and specialty-specific clinical practice guidelines.
Questions are distributed across the following 16 domains of advanced health assessment: comprehensive
health history taking; physical examination of the general survey, skin, HEENT, and neck; cardiovascular
assessment; respiratory/thorax assessment; abdominal assessment; neurological examination;
musculoskeletal assessment; genitourinary and breast assessment; mental health and substance use
screening; functional and cognitive assessment in geriatrics; dermatological assessment; diagnostic
reasoning and differential diagnosis; pediatric/lifespan assessment; cultural humility and health equity;
documentation standards; and legal/ethical standards with interprofessional collaboration.
This exam is intended for graduate-level MSN and DNP students and reflects the knowledge, skills, and
clinical judgment expected of advanced practice registered nurses (APRNs) entering clinical practice.
Students are encouraged to use this document as both an assessment tool and a comprehensive study
reference for national certification preparation.

Domain 1: Comprehensive Health History Taking
This domain covers the systematic approach to obtaining a complete health history, including the history
of present illness (HPI), past medical/surgical/family/social history, review of systems (ROS), and
functional assessment. Advanced practice nurses must demonstrate proficiency in using validated
screening tools (OPQRST, CAGE, AUDIT-C, HITS) and applying motivational interviewing techniques
per AACN Essentials and USPSTF guidelines.
Question 1. A 58-year-old female presents to the primary care clinic reporting "I've been feeling more
tired than usual for the past three weeks." Which follow-up question best exemplifies the OPQRST
framework for exploring the history of present illness?
A. "When was your last mammogram, and do you have a family history of thyroid disorders?"
B. "Can you describe what the fatigue feels like, what makes it better or worse, and rate its
severity on a scale of 0 to 10?"
C. "Have you experienced any unintended weight loss, night sweats, or changes in your bowel
habits recently?"
D. "Are you currently taking any over-the-counter supplements, herbal remedies, or prescribed
medications?"
Correct Answer: B
Rationale: This question directly applies the OPQRST mnemonic by addressing Quality,
Provocation/Palliation, and Severity. Bates' Guide emphasizes that OPQRST provides a structured
approach to characterizing symptoms, and the AACN Essentials require advanced practice nurses to use
systematic frameworks for history-taking.
Question 2. During a health history, a patient reports drinking "a couple of beers on weekends." Which
response by the nurse practitioner best employs motivational interviewing and the AUDIT-C screening
approach?
A. "I need you to be honest with me—drinking any amount of alcohol can seriously damage your
liver."
B. "How many drinks do you typically have on those weekend occasions, and how often do
you have four or more drinks in a single day?"
C. "You should cut back to no more than one drink per day as recommended by the Dietary
Guidelines for Americans."
D. "Since you only drink on weekends, your alcohol consumption is likely not a concern at this
time."
Correct Answer: B
Rationale: This response uses open-ended, nonjudgmental questioning aligned with the AUDIT-C
screening tool. The USPSTF recommends routine alcohol misuse screening using validated instruments,
and Bates' Guide stresses unbiased, patient-centered approaches when assessing substance use.
Question 3. A 72-year-old patient is being assessed for functional status. The NP asks about managing
finances, preparing meals, and using the telephone. Which classification is being evaluated?
A. Basic Activities of Daily Living (BADLs)
